Job Overview:
We are looking for a dedicated NDIS Support Coordinator to assist individuals with disabilities in navigating the National Disability Insurance Scheme (NDIS). In this role, you will work closely with clients, families, service providers, and other stakeholders to coordinate, implement, and review support plans that help clients achieve their goals and live independently.
Key Responsibilities: Assist clients to understand and access NDIS services. Develop and implement individualised NDIS support plans. Coordinate and monitor services to ensure timely delivery. Advocate for clients to ensure they receive appropriate services. Build relationships with clients, families, and service providers. Review and adjust support plans to meet changing needs. Assist clients with budgeting and funding management within NDIS guidelines. Skills and Qualifications: Knowledge of the NDIS and disability sector. Strong communication and interpersonal skills. Problem-solving abilities and a person-centered approach. Ability to manage multiple tasks and prioritize effectively. Experience in case management or support coordination (desirable).
