About the opportunity: The aim of the support coordination team is to provide assistance in coordinating the services and supports for National Disability Insurance Scheme (NDIS) participants, strengthen participants' ability to navigate the NDIS and to facilitate implementation of all supports identified in a participant's plan.
This position works to a standard consistent with the guidelines set out by the National Disability Insurance Agency (NDIA).
The role of the Support Coordinator is to support National Disability Insurance Scheme (NDIS) Participants to achieve their NDIS Plan goals, by coordinating:
Informal supports e.g.
connecting with family, friends and other community groups or networks.Mainstream supports e.g.
working with other sectors that have a responsibility to support NDIS participants e.g.
Health (hospitals); education (school, TAFE); housing; and justice;Funded supports e.g.
the funding for services outlined in a person's NDIS Plan, liaising with service providers who can deliver these supports.The role is performed in a competitive market environment where customer satisfaction with service quality and outcomes is essential for customer retention and attraction, and business sustainability.
Responsibilities include: Works equitably with Participants with respect to everyone's right to dignity, privacy, independence, and respect.Develop trusting, positive, and professional relationships with Participants and service providers.Provide information to Participants and advocate when necessary to facilitate access to community services and facilities.Support Participants in facilitating their independence by enabling empowerment and choice, and the promotion of personal resilience and social inclusion.Support participants with Plan Reviews.
Community engagement & education.Assists NDIS Participants to identify and plan strategies to achieve their personal goals as specified in their NDIS plans, within a person-centred framework.Liaises with carers, families and significant others involved in providing care, including specialist generic agencies and other mainstream service providers.Conducts community presentations to raise awareness of the NDIS, including people with a disability in mainstream support.Qualifications / Experience / Knowledge: Certificate IV in Community Services (Disability) or equivalent.A Degree in Psychology, Disability Studies, Mental Health, Education, or equivalent is highly desirable.Ability to use a range of computer software applications relevant to this role.Proven ability to work under broad direction.Well-developed analytical, research and conceptual skills.Fit to operate screen-based equipment.Current Victorian Driver's license.Knowledge & experience: Wide range of experience in developmental programming and service co-ordination.Experience in the development of Individual Program Plans.A clear understanding of Goal and Objective setting in relation to identified participant needs.Experience in identifying, developing, and evaluating communication matters.Experience in dealing and communicating with people with an intellectual disability.Skills: Knowledge of general Policies, Practices and Procedures, particularly as they relate to communication.Demonstrated knowledge of the Social, Community, Home Care and Disability Services Award 2010 (SCHCADS).Understanding of the HFCS Policies & Mandatory Procedures Manual.Good problem-solving skills.Proficient in the use of a range of computer programs i.e.
: Microsoft Word & Excel.Sound interpersonal skills with the ability to communicate with management, employees, and external participants/customers.Sound supervisory and organisational skills.Well-developed report and general writing skills.Knowledge of Occupational Health and Safety Act, Workcover Act, Equal opportunity Act.Good understanding of the operations and service agreements of HFCS.How to apply: If you think this sounds like you, we'd love to hear from you.
