The Role: The NDIS Service Coordinator role is a complex case management role, overseeing a caseload of up to 4 SIL's and 6 community clients.
The purpose of the role is to guide, mentor and support a team of workers to implement person-centred active supports to create an environment in which individuals can exercise their human rights and work towards achieving their goals, dreams and aspirations.
Reporting directly to the NDIS Service Manager you will:
Work collaboratively with a team of three other NDIS Service Coordinators to focus on supporting NDIS participants.Oversee the operations of supported independent living houses and teams.Ensure that participants and teams are working towards achieving NDIS goals and that supports are focused on building individual capacity.Ensure continuity of supports for individuals by ensuring that support schedules are within NDIS price guide requirements, budget constraints, and agreed upon services within the NDIS Service Agreement.Complete NDIS Progress Reports in line with participant NDIS Plan Reviews.Data reporting in line with individual plans and regular reporting requirements.Lead, coach, supervise and mentor staff on a day to day basis to develop skills and knowledge and ensure client needs are met.Be responsible for approving and monitoring client activity risk assessments.Regularly review and update client documentation to enable safe and effective support.Liase with and develop relationships with the team, external stakeholders, client families, allied helath professionals and support coordinators eg through stakeholder meetings and regular client team meetingsWork collaboratively with participant stakeholders to achieve positive outcomes.About you: You'll be driven and show genuine passion for building capability within your team. A natural leader, modeling a positive attitude, professional conduct and a passion and commitment to quality outcomes and service delivery for staff, individuals with disability and the community.Experienced leading a team and supervising the work of others.Enjoy working with a high performing group of colleagues and peers and accept responsibility and accountability for role.Have a positive track record supporting people with complex needs including neurodivergent, high physical support and complex health and behaviours.Self-motivated, well organised and resourceful.Possess a high level of intergrity, passion and reliability.Willing to live and work within the Mackay community.Technologically savvy and comfort utilising and learning through our apps eg rostering, incident reporting and client reporting systems.Possess relevant qualifications and/or experience in Community Services, Disability or related field.Significant experience and knowledge of NDIS and complex support needs.Experience with NDIS supports including working with neurodivergent and high physical support clients and complex behaviours.Significant experience in a leadership position within a community services organisation.Experience in working collaboratively with key stakeholders.Demonstrated administrative and technological skills and through knowledge of NDIS standards, Human Rights, Person-Centred Active Support and restrictive practices.The Offer: The NDIS Service Coordinator position is offered on a permanent basis with a competitive package including NFP/PBI salary packaging.
