About the Role
The organization is a host of home accommodation services which enable people with disability who want to live independently by providing flexible options for living arrangements and tailoring supports to match the customer's and family or carer's needs.
Within this role, your purpose is to provide support to our NDIS Participants including supervision to Disability Support Workers located within the residence(s) and coordinate within the team to ensure smooth delivery of the services.
This position will ensure the completion of applicable administration tasks whilst ensuring the quality of NDIS disability services for all customers. This will enable our participant's independent living in alignment with individual plans and the service offerings of the home portfolio (operating 24 hours per day, 7 days per week).
Responsibilities

Hybrid role involving administration, supervision and support hours.
Onboarding new clients and ensuring that all the required tasks are fulfilled.
Creating service agreements and updating these when required.
Liaising with support coordinators, families, clients, pharmacy, GP, OT, physio and other relevant team members to ensure smooth service provision.
Creating full client profiles in CRM and correctly entering funding into the system.
Creating service bookings on My Place portal for NDIA managed clients.
Supervising site operational activities, including client service delivery, site safety, customer care and safeguarding, site inventory and issue management.
Supporting the financial performance of the home service site through effective management of operating expenditure in line with budget.
Providing support to clients aligned to their personal plans, goals and any other requirements, including all aspects of personal hygiene and living skills to ensure a safe and comfortable living environment.
Supervising a small team on a rotating roster to maintain effective site operations and resolve issues.
Ensuring team member awareness of individual client's requirements.
Delivering and encouraging accurate documentation of all activities to ensure an accurate audit trail and maintenance of progress notes.
Medication management and behavior support supervision and monitoring.
Ensuring all client assessments are up to date and the organization is compliant in service delivery as per NDIS quality and safeguard commission.
Assisting with participant's physical and mobility requirements in accordance with training provided and relevant workplace health and safety standards, including transferring of participants.
Report writing for NDIS plan reviews and assisting with the review process.
Assisting in continuous improvement of the policies and documents related to client care needs.
Ensuring continuous smooth service delivery for clients and assisting with problem solving and completing all requested tasks from management.

Eligibility:

Certificate III in Disability and/or Certificate III in Individual Support or any relevant qualification to the related sector is desirable.
Demonstrated ability in assisting vulnerable people with relevant experience to identify goals and in assisting them to make the necessary choices and decisions to achieve these goals.
Demonstrated understanding of disability services provision and NDIS Quality & Safeguarding Framework or the ability to develop.
Ability to supervise site operations and deliver services aligned with organizational policies and procedures.
Ability to manage conflict whilst remaining calm and alert.
Flexible and able to work under pressure.

Job Type: Full-time
Pay: $80,000.00 – $85,000.00 per year
