Access Lifecare is a significant community-based social enterprise with the core purpose of enhancing the lifestyle and independence of adults with all disabilities.
Access Lifecare provides services to people with disability, providing meaningful opportunities to enable participants to realise their hopes and dreams through services such as accommodation, social and recreational activities, support coordination, recovery coaching and medication supervision. Our programs are designed to empower and develop independence and social skills.
The Role: We have a newly created opportunity for an Outreach & Activities Coordinator to join our ACT team. We are seeking a dynamic and energetic individual to provide, plan & coordinate group activities, manage our outreach staff, engage with internal and external stakeholders to influence a high level of service for clients accessing our programs and services. This is an immediate vacancy so submit your application today on Seek or for more information email us at ****** Key Responsibilities: Develop and implement the organisations outreach and activity programs across the ACT.Plan and organise group activities that promote social, recreation and skill-building opportunities for our participants.Work closely with participants to tailor activities that align with their personal goals and interests.Take a solutions-focused approach to adapt program changes in-line with participants needs to ensure engagement and meaningful experiences.Facilitate the delivery of high-quality clinical outreach services for eligible clients by ensuring referrals are appropriate, timely and accompanied by support plans.Direct provision of high-quality clinical services including initial intake, assessment and referrals.Organise and conduct initial intake assessments to ascertain care needs, behavioural issues and diagnosis to assess preliminary suitability of Access Lifecare.Develop and maintain positive relationships with participants and in achieving their individual goals, explore their practical and complex needs.Maintain accurate records of outreach services and activity programs.Support participants in accessing mainstream support e.g. mental health, legal, financial, counselling services, community and recreational connections, etc.Be responsible for the training, development and management of outreach staff.Lead, mentor, support and supervise staff and role model Access Lifecare's values and vision.Ensure clear internal communication strategies are in place to support the effective conveyance of organisational direction, processes, staff or participant concerns and compliance requirements between management and front-line staff.Develop and maintain effective communication channels with outreach staff, participants and their families/guardians to ensure successful outcomes.Foster a supportive and inclusive environment that promotes skill development, social interaction and well-being.Provide quality responses to feedback and complaints.Liaise with multiple teams to ensure support staff are matched with participant needs.Provide monthly reports/support letters to coordinators and guardians.Ensure all policies, procedures and legislative requirements are understood and followed by outreach staff and monitor compliance.Report hazards and incidents immediately, in-line with organisational procedures.Investigate Incident Reports in accordance with NDIS guidelines.Minimum Qualifications: Qualifications in Disability, Health, Community Services, Nursing, Social Science, Mental Health or equivalent.NDIS Screening Check (state based).Current driver's licence.Valid work rights in Australia.Current National Police Check (within 12 months validity), (or be prepared to obtain).Current First Aid & CPR certification (or be prepared to obtain).NDIS (National Disability Insurance Scheme) Worker Orientation Module Certificate.In-depth knowledge of the disability sector including NDIS.Demonstrated capacity to manage crises & critical issues and deal with complex & challenging situations.High level of organisational and time management skills.Ability to work effectively with a range of stakeholders including developing and maintaining relationships.Ability to work effectively as part of a diverse team.Excellent oral and written communication skills with the ability to communicate effectively with people from diverse backgrounds.Strong computer and administration skills.Willingness to be on-call.What We Offer: Access to our Employee Assistance Program (EAP).Ongoing support and advice from our experienced and friendly office and management team with regular follow-ups.Work/Life balance.Access to company vehicle.Full-time Position.If you are passionate about making a difference in the lives of people with disabilities and have the skills and experience required, we encourage you to apply.
