Join a purpose-driven organisation that values your growth and personal development and strives to make a difference in the lives of People with Disability.We are looking for a Part Time NDIS Coordinator of Support in Moora and surrounds to join our team.A Role with Impact:As a Coordinator of Supports, you will work with NDIS customers to build capacity to get the most value from their NDIS plans. This involves working with NDIS participants to bring their plan to life, providing various levels of coordination and transition support. You will link participants with service providers and allied health supports working towards them achieving their goals. You will be expected to travel and work in the Wheatbelt regions of WA.Selection Criteria:Exceptional interpersonal skills including the ability to work with customers, service providers, and various stakeholders.Knowledge and understanding of the National Disability Insurance Scheme (NDIS) and NDIS Practice Standards.Experience in case management and coordinating individual plans by utilising a range of community-based supports and methods to achieve customer goals.Effective management of complex situations/people and issue resolution with minimal support from management.Experience in working with people from diverse cultural backgrounds.Demonstrated experience in providing creative, flexible, and person-centred support to people with disabilities.A contemporary approach to individual customer choice.Well-organised with the ability to manage competing timelines.Strong communication skills, written and verbal.Collaborative and collegial team player as well as motivated to work independently.A current WA Driver's licence.A current National Police Clearance - or ability to gain one.A current Working with Children (WWC) card - or ability to gain one.A current NDIS Worker Screening Check.Knowledge and experience of living and/or working in the Wheatbelt is an advantage.
