What's the role?The NDIS Community Coordinator is responsible to manage and grow the NDIS funded disabilities services across the region with attention on managing an end-to-end therapeutic care service. The role will work in collaboration with the community's clinical employees to ensure an all-inclusive focus on the psychological and physical wellbeing of each participant. The role will consult with participants, their carers, families and guardians to ensure that they have choice and control regarding the supports they receive. Key focus areas of the role are participant care satisfaction, operative management of the Community Wellbeing Centre, program budgets and compliance, administration, incident management and employee support and management. 

In consultation with the Corporate Operations Manager, develop a strategic program growth plan to ensure engagement needs across the Sunrise footprint are met and initiate delivery of the plan.
Provide culturally sensitive and respectful leadership in the day-to-day management of the programs team members.
Provide an active and effective conduit between the NDIA, the relevant Sunrise communities, and its service provider agencies.
Support staff according to WHS requirements and social and emotional support when required.
Effective operational management of the Wellbeing Centre and program assets.
Take the lead in management of person centred supports for all participants and liaise regularly with families and participants to ensure delivery of evidence-based practices
Maintain appropriate administrative records utilising the NDIS data system in a timely manner
Use the Communicare System to record appointments, defined data fields, notes and actions to ensure the development and maintenance of a comprehensive single electronic file for each participant.
Maintain appropriate administrative records as per the policies and procedures of the program
Ensure that all reporting requirements are met by being recorded

Is this the right role for you?

Bachelor of Nursing or studying toward with a minimum of Enrolled Nurse
Qualifications in Mental Health Nursing (or comparable experience)
Experience in remote Aboriginal and Torres Strait Islander health care delivery
Current Driver's license
Current National Criminal History Check
Current OCHRE clearance
Acceptable vaccination status to work with vulnerable people in the NT
Right to work in Australia
AHPRA registration with no restrictions

NT PHN/RWA NT may be able to offer relocation support and clinical educational financial assistance to health professionals who are relocating to the NT or moving within the NT to a greater remoteness area classification for the purposes of employment and further clinical education. Want to find out more about this opportunity?
