We are now looking for an Integrated Health Manager to lead a dedicated team in delivering coordinated care and integrated health services across multiple community health programs.Role:The NDIS Community Coordinator will manage the end-to-end therapeutic care services for NDIS participants within the region.
You will be based out at Community during the week working closely with clinical staff, ensuring a holistic approach to participants' psychological and physical wellbeing, while promoting their autonomy and choice in the supports they receive.
You will oversee the daily operations of the Community Wellbeing Centre, manage budgets and compliance, and provide leadership to staff members.Key Responsibilities:Collaborate with the NDIS Team Leader to develop and implement a strategic growth plan for NDIS services across Sunrise communities.Provide culturally sensitive leadership and day-to-day management of the program team.Act as a key liaison between the NDIA, Sunrise communities, and service provider agencies.Monitor and manage risks, conflicts of interest, and any identified issues within the program.Ensure compliance with Work Health and Safety (WHS) regulations and offer support to staff as needed.Oversee the operational management of the Wellbeing Centre, ensuring all assets are effectively utilized.Lead person-centred care planning for each participant, working closely with families to ensure the delivery of evidence-based practices.Assist with medication management (if a registered clinician) in accordance with participants' plans.Maintain accurate and timely records in the NDIS data system and Communicare to support coordinated care.Ensure all reporting requirements are met and activities are well-planned and coordinated.Collaborate with Sunrise program staff during community visits and participate in capacity-building activities.Foster strong relationships with community and disability sector partners to strengthen program outcomes.Participate in audits, quality improvement activities, and ensure compliance with NDIS standards.Provide orientation and ongoing support for new staff members.Maintain program supplies and ensure compliance with program policies and procedures.What You'll Need to Succeed:Bachelor of Nursing or enrolled nurse, or currently studying toward a nursing qualification.Qualifications in Mental Health Nursing (or comparable experience).Unrestricted driver's license.Non-adverse National Criminal History Check.Current OCHRE clearance.Vaccination status compliant with NT health sector requirements.Right to work in Australia.AHPRA registration with no restrictions.What You'll Bring to the Team:Demonstrated experience in NDIS, mental health, or disability services, preferably in an Aboriginal community setting.Active participation in program audits and managing program compliance.Proven experience in managing a team and contributing positively to workplace culture.Strong organisational skills with the ability to plan, prioritise, and meet deadlines.Knowledge of local community networks and an understanding of social inclusion barriers.Proven ability to build and maintain collaborative relationships with participants, families, and stakeholders.Why Join Us?This is a rewarding leadership opportunity where you will play a pivotal role in shaping the health outcomes of remote Aboriginal communities.
