Infinity Care is a leading provider of in-home lifestyle and social support services in Australia. We are currently seeking NDIS Care Manager residing in the Melbourne Metropolitan area.
Position Overview: We are in search of a seasoned and passionate community care professional to offer independent, continuous advice and guidance to our clients across Melbourne. This is a full-time permanent position.
As a Client Adviser, you will collaborate with Infinity Care clients, their caregivers, and representatives to craft flexible, personalized, and culturally appropriate care plans. In this dynamic team environment, your responsibilities will include:
Reviewing intake information, engaging new clients, establishing home care agreements, crafting care plans, budgets, and connecting clients with services and supports. Conducting mobile work, visiting clients as required, implementing person-centered care planning, and providing independent advice and guidance to clients and their families. Collaborating with clients, caregivers, and representatives to ensure their ongoing needs and preferences are supported for independent living at home. Liaising with Infinity Care employees, stakeholders, and partnered service providers on behalf of the client and Infinity Care. Conducting annual reviews for all clients, revising care plans, goals, services, and budget expenditure. Ensuring ethical, fair operations within statutory, legal, and contractual requirements. Attending Head Office for training and team meetings. Qualifications: Tertiary qualifications in allied health, social science, health/welfare discipline, or a related field. In the absence of completed tertiary qualifications, experience in the delivery of person-centered practices in non-complex and/or complex case management, and/or demonstrated experience with complex service-oriented offerings in Disability services. Additional Requirements: All appointments are contingent upon a satisfactory National Police Check conducted by Infinity Care and a valid Working with Children Check provided by the applicant. A statutory declaration confirming residency in a country other than Australia is required. Depending on the information provided, international police checks may also be required.
