NDIS Area Coordinator - Brisbane South & North  About Us: Lifest HealthCare, a dynamic and growing NDIS Service Provider, is dedicated to delivering exceptional disability support services throughout Southeast Queensland. We are committed to helping our participants achieve independence, inclusion, and a greater quality of life through dedicated, hands-on care that complies with NDIS standards. We are currently looking for experienced Area Coordinators to join our team full-time (Monday to Friday). In this role, you will participate in and oversee the delivery of home and community disability supports for our participants and supervise a team of Support Workers across both the South and Northside of Brisbane. Key responsibilities include: Work directly with participants on the South and Northside of Brisbane to provide community and home care assistance in accordance with individual care plans and NDIS Goals, tasks will include assistance with personal cares, capacity building activities, monitoring and administering medications, organizing and attending medical appointments, promoting social and community inclusion and compliance reporting.Leadership & Supervision of a team of disability support workers, including training and ongoing mentoring. Rostering - coordinate the fortnightly allocation of support workers to participants based on participant care needs/hours and staff availability.Monitor the delivery of support services delivered to participants through review of timesheet submissions, shifts notes, behaviour and incident reports and any other relevant documentation.Respond promptly to participant feedback, concerns, or complaints, and take appropriate action to address issues and improve service quality.Ensure care plans and compulsory reports are maintained for each Participants and kept-up-to-date.Collaborate with external stakeholders including, support coordinators, health professionals, and other relevant parties.Communicate effectively with participants, their families, and relevant stakeholders regarding care updates, changes, or other relevant information.Our ideal candidate has:  A Diploma or Degree in Community Services (Disability) or a Bachelor of Social Work or equivalentA minimum of three years' experience in the Disability sector - working directly with participants with mental health and complex behaviours Previous experience in the NDIS industry held in high regard Previous experience in managing large and diverse teams held in high regardHigh-level of interpersonal and communication skills (written and spoken), with the ability to communicate effectively with staff at all levels of the organisationRelevant valid compliance documents: NDIS Worker Screening Card, First Aid, CPR, Medications training etcFull working rights in Australia - please note this role is not suitable for student visa holders.Valid driver's license with own vehicle with comprehensive car insurance (frequent driving is required)Proficient with technology Why Join Lifest HealthCare?
