NVC Group is seeking a Home and Community Care Coordinator responsible for overseeing the Home and Community Programs including Home Care Packages (HCP), Commonwealth Home Support Services (CHSP), flexible care packages, private home support, and brokerage services in our Kempsey community.
As a Not for Profit (NFP), NVC Group enables part of your salary to be packaged into pre-tax dollars to pay for personal expenses. Salary sacrifice can be applied to a vehicle, mortgage, rent, or EML card.
This immediate start position is based at our office in South Kempsey.
Key Responsibilities: Excellent communication skills (both written and verbal) and the ability to build community networks to enhance service delivery to our clients. A sound level of computer skills with the ability to undertake administrative tasks. Skills/experience to effectively coordinate for people with complex support needs by identifying and engaging successfully with various service types. Ability to manage competing tasks simultaneously and provide excellent outcomes in a cost-efficient manner. Ability to be self-motivated and work with limited direction. Commitment to continuing education and professional development. Experience and knowledge of community services. Tertiary qualifications in health care or management preferably with an aged care focus. Membership in any Industry Reference Groups, Committees, or Associations.
