About the Role

Multicap Tasmania has grown significantly and we now require a superstar employee who will come on this journey with us to start new services for clients and support new Disability Support Workers in Hobart. This is an opportunity you don't want to miss!
We are looking to add talent to our team within the Individual Supports area. You would play a vital role within the team while developing your own skills working alongside recognised peers and mentors. Your role would include:
The development of client centred support plans, developed from engagement with all team members, and using relevant assessments from external services and relevant stakeholders. Engaging with all team members to review the effectiveness of support plans and to identify opportunities for improvement in the planning of future service delivery. Ensuring the effective implementation of support plans for assigned clients, with a view to maximise the outcomes for clients and improved independence. Ensuring that all team members are provided regular and ongoing documentation of client's progress towards identified goals and collecting appropriate evidence as required. Monitoring each client to pro-actively identify any emerging support requirements and ensure that timely reviews of service delivery occur as required. About You

Your skills and experience might include:
An appropriate certificate relevant to the work required to be performed; Will have attained previous experience in a relevant industry, service or an equivalent level of expertise and experience to undertake the range of activities required; Appropriate on-the-job training and relevant experience; Entry point for a diploma without experience.
