The position leads and monitors the delivery of high-quality, evidence-based care for individuals with diagnosed mental illnesses and co-morbid chronic conditions. With a focus on Primary Health Care (PHC) and outreach, the role implements Stepped-Care services and is embedded in the PHC service model to promote holistic health care solutions for clients.Successful applicants will have access to a range of benefits, including:$119,634.00 - $128,524.00 + Superannuation11.5% SuperannuationAbility to Salary Package up to $15,900pa plus a further $2,650pa on Meals and EntertainmentThe ideal candidate will have a strong understanding of and commitment to Aboriginal Community Control and self-determination, as well as knowledge of the unique challenges faced by Yolngu people in remote settings. The candidate must have experience in delivering coordinated mental health services and culturally adapting evidence-based interventions for Aboriginal communities. A minimum of two years' experience in Indigenous mental health and alcohol and other drug services, along with a current Ochre Card, driver's license, and AHPRA registration as a Registered Nurse or Aboriginal Health Practitioner, is required.
