You will be working in the community with older adults experiencing mental health issues who may be living in their own homes, with family, or in RACF.
You will be providing recovery-oriented, person-centred biopsychosocial care planning and interventions reflected in all aspects of your practice and service delivery.
You will have previous experience working with older adults in a health or community setting, including experience of assessment, care planning and interventions which are appropriate for and effective with older adults experiencing mental ill health.
Previous experience in a care coordination role will be of benefit.

What You'll be DoingThe Mental Health Professional is responsible for providing evidence-based, strengths focused, recovery orientated assessment and treatment within a multi-disciplinary team and coordinating consumers' recovery plans.
The role is also responsible for non-clinical tasks including participation in quality improvement activities, clinical supervision and participation in administrative responsibilities as required Where You'll Be Working South Western Sydney is rapidly growing and is a great place for young people and families to build a life.

Relevant qualifications as a Registered Nurse, Occupational Therapist or Psychologist currently registered with the Australian Health Practitioner Regulation Agency (AHPRA), or qualified Social Worker eligible for registration with the Australian Association of Social Workers (AASW), or CSU Graduates: Bachelor of Health Science (Mental Health).
Please provide a detailed description of your interest and motivation in applying for this role highlighting your relevant skills & experience in providing assessment and treatment for consumers of the mental health service.
Using an example, describe a time where you have had to work as part of a team to solve a problem.
