Bridges Health & Community Care Ltd. is looking for full-time Mental Health Practitioner based in Rockhampton, QLD.
In this role, you will work alongside individuals experiencing mental health challenges, using evidence-based interventions and recovery frameworks to support them in their recovery journey.
Reporting to the Regional Manager, you will be responsible for helping individuals develop recovery goals and providing culturally appropriate, person-centred services.
This role requires travel between Yeppoon, Rockhampton, and Gladstone to ensure the seamless delivery of support and services across these locations.
As part of this position, you will engage in extensive outreach activities, fostering and maintaining strong relationships within diverse communities.
A company vehicle will be provided to facilitate this travel.

The key role of the Mental Health Practitioner is to support people with moderate to severe mental illness who have been incarcerated, to facilitate their transition back to community.
The incumbent will be responsible for providing recovery focused mental health services to people experiencing moderate to severe mental illness, including providing support to access the National Disability Scheme (NDIS), and providing referrals to appropriate services.
More specifically, your responsibilities include but are not limited to:
Providing intensive support for individuals to develop meaningful personal recovery goals Offering support services with a focus on social justice and human rights principles Assisting individuals in accessing long-term support through the NDIS and local resources Collaborating with allied health, disability, and drug and alcohol services to ensure integrated service delivery using the "No Wrong Door" approach We strongly encourage Graduates and experienced professionals to apply for this role.
About You To qualify, you will need at least a Certificate IV in Mental Health, with qualifications in health, social, or behavioural sciences highly regarded.
Experience in working with diverse populations, including Aboriginal and Torres Strait Islander people and marginalised communities, is highly valued.
A working understanding of the NDIS will be a significant advantage.
