About the opportunity
An exciting leadership opportunity to work with a dedicated Community Mental Health Team to provide excellent holistic mental health care incorporating physical health needs of the mental health clients.
The role of the Mercy Mental Health (MMH) Nurse Practitioner (Physical Health) is to improve the coordination and outcomes of the physical health care of consumers living with co-occurring chronic physical health conditions.
The MMH Nurse Practitioner (Physical Health) will work towards improving the health outcomes of consumers by promoting health screening, assessment, treatment, care coordination, capacity building and linkage to primary and acute health care.The MMH Nurse Practitioner (Physical Health) will support clinical education, mentorship and offer innovative programs to enhance engagement of consumers in therapy and treatment and provide extensions to practice through prescriptions of medications, ordering of diagnostic investigations, direct referral to other health care professionals within the scope of their AHPRA registration as an Endorsed Nurse Practitioner.The role will support the following components:Stewardship and care coordination of a caseload of community mental health clinic (including Community Plus) managed consumers with multiple physical health co-morbidities who access multiple parts of the health care system (such as different acute health departments, emergency departments and outpatient services), aimed at improving treatment continuity across settings and between psychiatric and general health services.Primary physical health assessment and treatment of referred consumers who are not engaged with a regular GP and who require short term treatment, intervention and primary care linkage.Staff capacity building, education, secondary consultation and contribution to clinical guidelines and policy development.What you'll bring

Division 1 Registered Nurse with Postgraduate qualification in Mental Health NursingCurrent endorsement by the Australian Health Practitioners Regulation Agency as a Nurse Practitioner, having completed a Masters of Nurse Practitioner (or a Masters granted equivalence of qualification by the Australian Health Practitioner Regulation Agency plus an approved Therapeutic Medication Management module).
Desirable:Substantial clinical experience in community and acute mental health settings, minimum 5 years in a senior nursing roleRelevant experience in clinical assessment and treatment in the areas of mental health care and chronic disease management.Evidence of competent utilisation of extensions to advanced nursing practice according to approved Clinical Practice Guidelines in mental health and chronic disease management.Membership with the Australian College of Mental Health Nurses or the Australian College of Nurse PractitionersFamiliarity with the concept of Equally Well Victoria - a framework to address comorbidities.Certificate IV in Training and Assessment.A current valid Victorian Driver's licenseTo thrive in this role, you will have:
