Support clients within a community based setting to provide holistic and coordinated care. Development and review of client care and treatment plans to effectively support and monitor wellbeing and treatment goals. Provide Brief psychosocial Interventions for clients. Work collaboratively with multidisciplinary team members to support clients with their health and wellbeing goals. Support the establishment and connection with additional health and mental health professionals, as required. Advocate and support client's health and wellbeing in collaboration with their treating health professionals. Provide holistic, wrap-around support and education across all aspects of a client's health and wellbeing. Requirements: Registration with the Australian Health Practitioner Regulation Agency (AHPRA) with a current annual practicing certificate (essential). Current credentialing as a Mental Health Nurse through the Australian College of Mental Health Nurses (desirable) Extensive experience working in mental health settings and managing clients with severe and persistent mental health conditions and complex needs. Highly developed communication (including written) and interpersonal skills, The ability to engage people who are hard to reach and advocate on their behalf Establish and maintain effective relationships with staff, clients, carers and stakeholders. Ability to use a desktop computer, with familiarity with Microsoft Office applications and capacity to learn how to use a new clinical software package Must be eligible to apply/currently hold a Blue Card Must hold/be willing to apply for NDIS worker screening clearance Current Driver's License.
