The Role To enhance the effectiveness and utilisation of General Practitioner (GP) resources with the aim to improve access to healthcare for patients in Justice Health and Forensic Mental Health Network (JHFMHN) that have associated mental health issues as per the Mental Health Model of Care by providing an expert patient centred nursing assessment and care coordination of patients throughout the state.
The CNC participates in patient care provision to patients state-wide and actively liaises with JHFMHN General Practitioners; Psychiatrists, CNC'S and nurses across primary care, mental health and drug and alcohol.
