South West Healthcare is seeking a Mental Health Clinician to lead the HOPE team, delivering post-suicidal crisis support. This role involves managing case allocations, developing care pathways, expanding services, and providing clinical supervision, all aimed at empowering clients through follow-up care and connection to resources for up to 12 weeks.
About the Role South West Healthcare is looking for a dedicated and skilled Mental Health Clinician to lead our Hospital Outreach Post-suicidal Engagement (HOPE) team. This team, part of our Mental Health and Wellbeing Directorate, offers non-clinical assertive outreach support aimed at empowering individuals by connecting them with both informal and formal resources. Our focus is on providing guidance, encouragement, and follow-up care for individuals after a suicide attempt or during a suicidal crisis, with the goal of reducing the risk of further attempts by supporting them for up to 12 weeks.
In collaboration with the Manager of Referral Services, the HOPE Team Leader will manage case allocations, establish care pathways, oversee service expansion initiatives, and provide clinical supervision to team members.

The funds can be applied toward rent, childcare, relocation expenses, and more.
What You Bring As a health professional, you will possess extensive experience in mental health clinical services and a strong ability to work effectively in a team-based, multi-disciplinary environment. A solid understanding of recovery-oriented practices, trauma-informed care, and current mental health trends, along with knowledge of relevant legislation and policies, is crucial for this role.
In addition, you will have:
