About the service: Head to Health is a community mental health and wellbeing service for adults, their families and carers that offers access to integrated care and support to improve recovery outcomes. Participants can access a range of community mental health support services; clinical mental health services; care coordination.

Conduct risk assessments, including assessment of suicide risk, violence/aggression and vulnerability and provide risk management in collaboration with the individual, their family/carers and other support systems with active engagement of appropriate referrals and treatment planning.Requirements: Must have one of the following: Full AHPRA registration as an Occupational Therapist or Mental Health Nurse, General or Provisional AHPRA registration as a Psychologist/Clinical Psychologist OR AASW registration/eligibility as a Social WorkerKnowledge & understanding of the theory underpinning evidence-based interventions & clinical practice as well as demonstrated excellence in the practical application of such, in the areas of mental health and alcohol and drug use.Highly developed communication (including written) and interpersonal skillsThe ability to engage with people, advocate on their behalf and establish and maintain effective relationships with staff, clients, families & friends and stakeholders.Experience in the use of electronic client/customer management systems, ability to use a desktop computer, and experience with Microsoft Office applicationsMust be eligible to apply/currently hold a WWCCMust hold/be willing to apply for NDIS worker screening clearanceStride is committed to the safeguarding of children and vulnerable people.
