Medication Safety Pharmacist

40 hours per fortnight Fixed Term Part Time until 30 June 2025 Grampians Health is seeking a motivated hospital pharmacist with established clinical and leadership skills to work collaboratively with the Lead Pharmacist - Medication Safety to co-ordinate the medication safety program across Grampians Health, a large and diverse multi-campus regional health service.
The position is for a fixed term, replacing the current Senior Pharmacist - Medication Safety, who is being seconded to another position.
The successful applicant can choose any Grampians Health campus as their primary work location, but the role requires a whole of Grampians Health focus.
Key Responsibilities: Develop and implement system-based, consumer-focused medication safety quality improvement strategies to reduce the risk of harm to patients and enhance medication safety Monitor and evaluate medication safety systems through audit and incident report investigation, analysis and reporting Provide medication safety related training and education to clinicians and consumers, as required Skills and experience: Currently hold general registration as a pharmacist with the Australian Health Practitioner Regulation Agency (AHPRA) At least 5 years hospital pharmacy experience, including broad exposure to the different aspects of the medication management pathway Demonstrated ability to effectively engage stakeholders to achieve desired outcomes Demonstrated ability to effectively plan, organise, prioritise and manage time with ability to deliver outcomes to agreed timelines To see a full copy of the Position Description please  click here.
