About the facility:Our major sites include University Hospital Geelong in Geelong's CBD, the McKellar Centre precinct in North Geelong, Barwon Health North in Norlane, and residential aged care facilities spanning the region.
Comprehensive mental health, community health, and dental services are delivered through various programs across the Barwon South West Region, including Torquay and Colac.
About the Connected Care North Program:Connected Care North is a new program established in 2023, working initially in the northern suburbs of Geelong, but has extended to include the Barwon Health catchment.
The inter-professional team works with clients with chronic and complex health conditions to decrease hospital demand through the provision of comprehensive assessment, care coordination, and support to access specialist care in the community.
The model of care is responsive and agile to client and organisational needs.
The team includes care coordinators, nurses, medical staff, and allied health professionals including psychology, diabetes educator, social workers, and pharmacist.
About the role:This role is to provide medical leadership, assessment, and treatment to clients in the program.
The role is intended to support the planning, implementation, and organisation of care aligned with the Barwon Health Connected Care model of care.
Work will include direct client care and secondary consult supporting staff within the team and other services including GPs.
There is also an opportunity to provide input into the developing agile model of care, ensuring that the care provided achieves excellent health outcomes for those in the Northern area with chronic physical and behavioural health conditions as well as social barriers.
