We have an exceptional opportunity for a Medical Case Manager to join our medical team. As a member of our medical team, you will be delivering high-quality healthcare services to children and their families.
Job Locations: Big Springs Texas, Midland Texas, and Donna Texas.
Your responsibilities may include, but are not limited to: Listening to concerns and providing counseling or intervention as required. Evaluating clients' progress periodically and making adjustments as needed. Providing support to put care plans in place. Requirements: Education: Associate degree or Bachelor's Degree in Nursing, Social Work, Human Services, or related field from an accredited university. Master of Nursing, Social Work, or in a social services field is preferred. Experience: Minimum of five (5)+ years of current experience in case management and diverse hospital/clinical experience working with children and families. Previous utilization management, discharge planning, or hospital-based social service experience is preferred. Certifications: Must hold a current Basic Life Support (BLS) certification. National Case Management Certification (CCM or ACM), Licensed Master Social Worker (LMSW), or Licensed Clinical Social Worker (LCSW) if holds an MSW degree. Must have experience working with and be comfortable with young children's and adolescents' medical, behavioral, and social needs. Type of Contract: W-2 or 1099 Hours per week: 40 per week (this could alter pending on operational needs) Benefits: Health, Dental and Vision, 401K plan, 401K matching, paid holidays, paid time off.
