The Role The McCusker Nurse offers expert advice, support, and referrals to carers of people living with dementia in metropolitan Perth, regardless of their aged care provider. They act as a key resource, providing guidance and fostering connections with dementia specific agencies to provide support.
This position is being offered on a Permanent Part time basis working 45.6 hours per fortnight from 9:00am to 5:00pm at our Head Office in Subiaco. This roster is flexible covering 3 nominated days each week.
Key responsibilities: Provide dementia-specific person centred, information, education, and/or counselling, including telephone, written and face-to-face communication.Provide expert advice, and resources to support the carers of people living with dementia.Establish and maintain positive and professional relationships with people living with dementia, their carers, and the broader community.Provide advocacy, and support for people living with dementia and their caregivers.Ensure a timely and seamless referral process for clients to external and internal service providersImplement a regular survey which will collect, and report on outcome measure data from the service providedMaintain client record management and ensure reporting is in accordance with Amana Living's policies and procedures.Where possible, provide education about dementia and McCusker Nurse services to Aged Care Service Providers, Aged Care Assessment teams, and other community groups.Present at professional conferences and workshops as requested, to promote and raise awareness of Amana Living's role in dementia services.About you The ideal candidate will have extensive experience in the aged care sector, particularly supporting carers of people with dementia. They will possess strong knowledge of dementia and related services, along with excellent clinical reasoning and decision-making skills. The candidate will be able to conduct telephone interviews, assess client needs effectively, and adapt to varying communication abilities. Proficiency in computer literacy, including Microsoft Office and other database programs, is essential.
