$80-90k plus company car, mobile, and laptop.


Responsibilities: Assist participants in understanding and utilising their NDIS plans to achieve goals, live independently, increase skills, and engage with their community and employment.Connect participants to allied health supports (OTs, Physiologists, Psychologists, Nurses, Dietitians), and link to community, mainstream, and government services.Monitor, guide, and assist participants in exercising choice and control and coordinate supports and reviews.Complete necessary forms and risk assessments per policy and procedure manual and code of conduct.Contribute to building a stakeholder network.Participate in meetings and share relevant training opportunities. Required:

2 years of experience in relevant case management/social work with a person-centred approach.

Masters degree in Social Work or equivalent.


Desirable:

Registration with AASW or ACA or a recognised body.

Experience in aged care/home care service/support as well as NDIS support coordination.
