We are currently seeking an experienced manager to work closely with our Executive Team to prepare and lead service delivery under our strategy plan as we adapt and respond to sector-wide Aged Care Support at Home reforms.
This role is critical in maintaining accessible, affordable, quality services that are responsive and support and enable optimal care recipient outcomes.
The Manager Home Care Package (HCP) Services will lead service design and, subject to successful provider registration, activation, building and overseeing a team of Care Advisors/Case Managers and Program Support Officers.

About the role:
This role will oversee the establishment and day-to-day operations, ensuring effective communication among healthcare providers, care recipients, and families and ongoing compliance with regulatory standards.
Responsibilities include:
Lead the operational establishment of the HCP team and services within BCH. Maintaining effective communication and collaboration among internal and external service providers and care recipients. Monitor and respond to service delivery metrics to ensure services remain responsive, compliant and sustainable. Identify and implement quality improvements. Contribute to change management planning and support the team and organisation through change and service integration and Aged Care Reforms. What you will bring to the team:
Qualification in one of the following disciplines:
Social Worker: An AASW accredited bachelor's degree that provides eligibility for membership of the Australian Association of Social Workers Psychology: A post-graduate degree in Psychology. Current Registration with Australian Health Practitioner Regulation Agency (AHPRA) as a Psychologist. Certificate or Diploma in Counselling Nursing: Registered as a Division 1 or Division 2 Nurse under the Australian Health Practitioner Regulation Agency (Nursing and Midwifery Board of Australia) or Member of Australian College of Mental Health Nurses Allied Health: Including but not limited to accredited bachelor's degrees in Occupational Therapy, Physiotherapy or other Allied Health Disciplines and Current Registration with Australian Health Practitioner Regulation Agency (AHPRA) or Exercise and Sports Science Australia (ESSA) Extensive experience as an HCP Care Manager or Team Leader, with in-depth knowledge and understanding of the Aged Care Quality Standards.
