Job DescriptionAbout the role The Family Wellbeing Manager actively leads the team in enacting wrap-around care for young people and their families across the ATSICHS system, including the broad range of Human Service programs, Medical, and Early Childhood services through analysis of systems that prevent activating an integrated service system.
Duties Lead the implementation of staff supports that maintain adherence to the program Model of Care, Human Service Practice Framework, and enhancing staff role clarity that places clients quality and impact as the primary focus.Lead the implementation of management operational processes and non-negotiable tasks.Lead one-to-one formal in-field coaching/training with each staff member, utilising the Mobcare coaching system and all relevant tools in the framework.Lead the transparent use, access and modelling of data for staff and team to see and understand on a regular basis. Ensure all team printed dashboards are updated monthly in common areas.Ensure leadership decisions are data informed and based on the priority of achieving client impact and quality.Ensure direct reports and team consistently use client outcome tools (Outcome Rating Scale) at each home contact.Ensure all case plans and interventions provided by team are meaningful outcomes based that do not promote client dependence on services.Lead the relationship development with Child Safety Service Centre Managers, ensuring programs and services are responsive to needs of partners.Directly and proactively manage team culture worries, ensuring changes are consulted with the Program Manager.Culture We are a diverse, ambitious team excited about what we do. Permanent full time position at Logan and Brisbane.Starting annual salary of $113,000 (depending on qualifications and experience).11.5% Super + Not for Profit Salary Packaging (Increases your take home pay).Work for a community-controlled organisation.Desired Skills and ExperienceCOVID-19 Requirement Immunisation history showing you have received two doses of a Covid -19 vaccine is encouraged.Requirement Current Bachelor's Degree in Social Work, Human Services, Social Science, Health, Allied Health, Youth Work, Community Services, Family Intervention or Criminology with 4 years' relevant experience in family support, child protection, youth support or related field.OR Current Diploma in Community Services, Social Work, Human Services, Child and Family, Allied Health, Youth Work, Child and Family Intervention with 5 years' relevant experience in family support, child protection, youth support or related field.AND With at least 3 years of abovementioned experience in a case management role.
Demonstrated ability and comfortableness working in a performance driven environment to meet individual weekly, monthly and quarterly performance targets.Demonstrated ability to build client agency, self-efficacy and facilitate change through client empowerment, while actively discouraging client dependence on supports.Demonstrated knowledge and ability to provide behaviour change interventions that build client self-awareness and active change in achieving their identified goals.Demonstrated ability to work with the whole-of-family, irrespective of primary client, in meeting the totality of family needs.Demonstrated ability in coordinating multiple services in providing access to holistic care for clients, particularly integrating internal agency supports across multiple teams.Demonstrated knowledge and understanding of the factors contributing to vulnerability, disengagement, and intergenerational trauma of client group.Experience with case management for high risk and/or crisis clients, with high-level planning and organisational skills.Knowledge of the supports available to target clients in the areas of mental health, substance misuse, education, employment and housing.Additional Requirements Requirement to travel across the Logan and/or Brisbane region, as per program location.Flexibility to work outside 8:30am-5pm business hours in meeting individual client needs and provide responsive care.Working with Children's Card (Blue Card) or be eligible to apply.How to apply If this sounds like you, please submit your application by following the links on the recruitment page.
