Join a dedicated, multidisciplinary team within the hospital's Community Services department, where you'll deliver vital physiotherapy care directly to patients in their homes across Regional Victoria.
What Makes This Role Special: Direct Community Impact: Play a key role in helping patients regain mobility and independence through the hospital's community-focused rehabilitation and restorative care programs.Engaging Variety: Step into a diverse role, working across programs like Rehab in the Home and Transition Care Programs (TCP), providing dynamic and compassionate care within a supportive hospital framework.Supportive Flexibility: Enjoy the independence of working in community settings with the full backing of the hospital's multidisciplinary team, all committed to delivering top-quality healthcare.Primary Responsibilities: Conduct comprehensive assessments and deliver targeted physiotherapy interventions designed to improve patients' quality of life.Provide hands-on, patient-centered care in Rehab in the Home and TCP settings, with a focus on functional movement, strength, and safety.Collaborate closely with a multidisciplinary hospital team, participating in case discussions and delivering holistic, coordinated patient care.Mentor Allied Health Assistants and junior physiotherapists, fostering skill development and a strong culture of patient-centered care.Champion quality improvements aligned with NSQHSS standards, helping to shape the future of these impactful hospital programs.About You: Qualifications: Bachelor's degree in Physiotherapy with general AHPRA registration.Full Australian working rights - Working Holiday Visas welcome!Demonstrated experience in managing caseloads independently within community-based programs.Strong communication and interpersonal skills for engaging effectively with patients, families, and the hospital team.Proficiency in organisation and digital documentation for efficient case management.Valid Australian Drivers License.Background in Rehab, Restorative Care, or Outpatient services, including Rehab in the Home and TCP.Experience supervising students or junior staff.This is a unique opportunity to bring your expertise to a regional public hospital and make a meaningful difference within a community-focused role!
