LME Care, a premier disability support provider in Brisbane, is looking for a dedicated and dynamic Community Access Coordinator to join our team. With over 25 years of combined experience in the disability sector in Australia, LME Care is committed to delivering high-quality, person-centered support that empowers individuals with disabilities.
As a Community Access Coordinator, you will play a crucial role in enhancing community participation and fostering independence for our clients. You will work collaboratively with individuals to identify their unique goals, develop access plans, and facilitate connections to community resources and activities that promote inclusion and engagement.
You will also be developing and expanding Community Access at LME, ensuring a top-notch Community Access experience by creating opportunities, building networks, and developing person-centered community programs.
Responsibilities Assist clients in identifying their interests and goals related to community access Develop individualized community access plans based on assessments and client needs Build and expand the Community Access section of LME Care Coordinate and facilitate participation in community activities, programs, and events Collaborate with families, support teams, and external agencies to optimize service delivery Monitor and evaluate the effectiveness of community access programs Provide guidance and support to clients in overcoming barriers to participation Ensure compliance with all relevant policies and procedures, including health and safety regulations Maintain accurate documentation and reporting on client progress and program outcomes Engage in ongoing professional development to enhance skills and knowledge in community access Minimum Requirements Experience in disability, community development, internal/external relations or related fields Certification in Support Work, Community Services or related field highly desired Confident in developing and maintaining positive relationships with clients, families, and community organizations Strong understanding of the challenges faced by individuals with disabilities and their families Excellent organizational, communication, and interpersonal skills Ability to market events, programs and community service opportunities Demonstrated ability to identify available services and service gaps Proficiency in creating and implementing individualized plans Problem-solving skills and the ability to navigate complex situations Current first aid and CPR certification Valid driver's license and access to a reliable vehicle Understanding of the NDIS framework and community resources available to individuals with disabilities Availability to work flexible hours including evenings and weekends may be required Remuneration SCHADS Level 4.1
