We are seeking a compassionate and reliable Lifestyle Program Support Worker to join our team in Port Pirie.
In this fulfilling role, you will deliver centre-based activities for clients across various programs, including the Commonwealth Home Support Program, Home Care Packages, and NDIS.
Your focus will be on providing social support, promoting independence, and creating a positive and inclusive environment for our Aged Day Program participants.
Key responsibilities:
Plan and deliver engaging activities for participants in line with relevant program guidelines.Provide social support that promotes independence and well-being.Support clients with transportation to and from day programs.Assist in the development of personalised Support Plans and leisure assessments.Ensure culturally appropriate service delivery and maintain accurate client records.Essential Criteria Completed AQF Certificate in Aged Care or a relevant qualification.Experience in delivering direct care services, particularly to aged, frail, or disabled individuals.Strong administrative skills, including proficiency with Microsoft Office.Ability to work collaboratively within a team.Compassionate and dedicated approach to client care.Ready to make a difference?
