About The Role And Where You Will Be WorkingWe have a great opportunity for an enthusiastic Lifestyle Coordinator for Jeremiah Donovan House in Forrestfield. If you are passionate about working in partnership with older people, supporting their well-being and maintaining their connection with community this could be the role for you.
As the Lifestyle Coordinator you will develop, implement and monitor an activity program that ensures individual resident needs, values, spiritual and cultural preferences are taken into account. You will champion resident choices, goals and independence. You will coordinate a team of Therapy and Lifestyle Assistant staff and volunteers, working collaboratively with Occupational Therapists, Physiotherapists and other health professionals to ensure provision of optimum level of person-centred service for all 65 residents.
This is a permanent full-time position reporting directly to the Facility Manager and an important part of the multidisciplinary team at Jeremiah Donovan House.
We'll Also Be Asking You ToDevelop and monitor lifestyle programs and drive the engagement and participation of residents in a range of social, recreational and cultural activities.Actively participate in the delivery of engaging and meaningful lifestyle activities.Complete accurate assessments and documentation of participation.Lead and coordinate bus excursions with residents accompanied by the bus driver.Support the orientation and training of new team members and volunteers.Build genuine relationships with residents and their families.Facilitate community engagement opportunities.About YouTo be successful in this role, you will have experience in lifestyle or allied health assistant services within residential aged care or a similar role and a strong commitment to person-centred care. Most importantly, you'll have a genuine desire to make a positive difference in our residents' lives.
To Be Considered For This Role, You'll NeedCertificate IV in a relevant area (preferred - Allied Health/Lifestyle/Leisure).Demonstrated leadership skills with at least 3 years of experience working in a lifestyle or similar role within residential aged care.Significant experience working with people living with dementia.Excellent verbal and written communication skills, be well organised and able to work effectively as part of a team as well as independently.Proficient computer skills using a range of programs and platforms including MS Office and Canva.Experience in meeting the Aged Care Quality Standards.Working With UsAt Southern Cross Care (WA), we are committed to being an employer of choice.
