An exciting opportunity exists to work for one of South West Queensland's most dynamic and community responsive organisations.
Lifeline Darling Downs and South West Queensland Limited (LDDSWQL) has been serving the community of Toowoomba, and the wider South Western Queensland area for over 40 years.
The Tracks to Success program aims to work with parents and families to support them in improving their capacity and engagement in the education of their children and young people.
This is underpinned by the knowledge that consistent engagement in education will provide better long-term outcomes for children and young people.
The Tracks to Success Program model provides a strengths-based case management framework, individualized to a child or young person within a family context.
Strategies used to achieve this work include brief initial assessment, referral, support, application of therapeutic intervention strategies, and the development and facilitation of psycho-educational groups for students and parents.
Permanent Part Time opportunity at a 0.6 FTE (22.8hrs per week)
Hours required on Wednesday until 5pm to support school program, flexible options for remainder of hours.
As our Tracks to Success worker, your duties and key responsibilities will be:
Develop and implement good case management practices including intake, assessment, intervention, and evaluation processes.
Initial and ongoing assessment of the needs of the child or young persons within their family and community environment.
Complete detailed and accurate case notes.
Support children, young persons and families to meet and overcome any identified barriers so improved and ongoing engagement in early childhood and schooling can occur.
Work collaboratively with other community agencies and link/refer families as needed.
Complete and provide reports and feedback to all stakeholders on the progress of the program.
Key Selection Criteria:
Experience working with Aboriginal and Torres Strait Islander children and families in a case management or community role.
Experience in managing cases, including intake, assessment, support, and review, for families dealing with tough and complex challenges.
Ability to work respectfully and culturally appropriately with families and services to meet both personal and program goals.
Skills in writing clear case notes and reports, and sharing feedback verbally with different audiences within tight deadlines.
Strong understanding of trauma and intergenerational trauma, and how this impacts children, young people, and families.
Good communication skills—both spoken and written—with a focus on building trust, resolving issues, and working together.
A current Queensland "C" class driver's license.
A valid Working with Children Blue Card.
This role will require our ideal candidate to show demonstrated ability to combine your lived experience as an Aboriginal and/or Torres Strait Islander person and understanding of Aboriginal and Torres Strait Islander ways of knowing, doing and seeing.
