As a Lifestyle Coordinator, you will provide person-centred care to our residents. You will work in conjunction with the consumer, Lifestyle Assistant, Facility Manager, and Clinical Manager to develop, plan, implement, and evaluate a range of unit-based activities in response to consumers' assessed and stated needs.
You will ensure each consumer has an individual lifestyle plan formulated with consumer and relative consultation that is reviewed and evaluated in line with the Unit's documentation protocol.
