The Salvation Army - Burnie Safe Space is a 24/7 facility, providing crisis accommodation and short-term shelter facilities for people experiencing primary homelessness across North Tasmania.The role of the Key Development Coach is to provide TSA Safe Space clients case and support planning towards meeting outcomes in the transition away from homelessness.
As a Key Development Coach, you will work in collaboration with TSA Case Managers and Partner Organisations in coordinating challenges to obtain housing.Engage with guests, clients and participants to develop rapport and trust.Provide immediate emotional and physical support/comfort to participants.
Make participants feel safe/wanted/comfortable while ensuring they understand the house rules to be able to use the shelter for the night: (operating in a harm minimisation environment, with no alcohol or drugs allowed on the premises)Assess each participant to ensure it is safe to provide shelter for the night; reviewing their safety and the safety of others if they are under the influence of drugs/alcohol.Provide participants with information about other support services they can access to assist them move into transitional housing/support.Review case files and input SRS and internal communications data.Providing ongoing assessment of clients' housing and health challenges, needs and circumstances from intake to support transition.Developing individual case plans in collaboration with clients that includes a range of interventions that address the needs and goals of clients.Assisting clients to access appropriate support services and establish community supports, and where needed ensuring clients are assisted through advocacy with service providers.Monitor and review progress, pathways, and outcomes of individual case plans and act where needed to ensure plans continue to meet client needs and goals.Identify risks or hazards to clients, staff and TSA and report to Leadership Team.Ensure the highest levels of confidentiality and privacy are maintained for each participant.Support clients with on- and off-site services engagement.Draft, Implement, and Review Support Plans.Ensure the highest levels of confidentiality and privacy are maintained for each participant.Support team members (staff/volunteers) so that a co-ordinated and collaborative approach is adopted from the outset.
Ensure highest levels of information exchange to support the best outcomes for participants and staff safety.Willing to obtain a good working knowledge of health and AOD services.Demonstrate a good working knowledge of referral and support options for participants.Work collaboratively and professionally with other services.
