Enjoy the benefits from joining a national network, whilst working locally. We are currently looking for a full time NDIS Support Coordinator to join our Mid North Coast team.
The role As a Support Coordinator, you'll be the vital link in enabling NDIS participants to achieve their goals. You'll help build their capacity, coordinate services, and manage a variety of supports while maintaining a high standard of compliance and quality.
Key Responsibilities
Research, coordinate and manage a range of supports to suit individual needs across multiple providers. Coordinate access to support items in an NDIS Participant's Plan in line with the preferences of the participant. Support participants to strengthen their ability to connect with informal, mainstream and funded supports. Support individuals to build their capacity and networks with the aim of greater choice, control and independence. Ensure all reports are written in accordance with the NDIS framework and in a timely manner. Assess mainstream, community and informal provider options and guide participants through their choices. Negotiate services and develop service agreements. Liaise with plan managers regarding funds and claim management categories. Ensure that service provision is informed by the participants' individual needs. Work with customers and their natural supports to assess the quality of the supports provided. Ensure quality standards are maintained and adhere to Just Better Care's Quality Management System. Respect the privacy and dignity of all participants. What You'll Bring
Proven ability to lead and inspire a team. A minimum of 2 years' experience as a Support Coordinator with a registered NDIS provider. A genuine interest and demonstrated ability to manage complex cases. Passion for delivering exceptional customer service and building trusted relationships. What you will need
Knowledge of the National Disability Insurance Scheme (NDIS) and the disability sector. Experience in providing services to customers including assessment and monitoring processes. Demonstrated high level verbal and written communication skills, including familiarity with the Microsoft Suite. Current and clear NDIS Worker Check. Evidence of COVID Vaccination.
