Join Junction Support Services as a Children's Case Manager! In this vital role, you will manage cases at multiple support levels, from initial assessment to enhanced case management, ensuring that each child receives tailored support. Your responsibilities will include:
Conducting comprehensive assessments Developing and monitoring flexible case plans Facilitating therapeutic group work Collaborating closely with referring agencies and external services Advocating for the child's best interests Enhancing family well-being Assisting with children's transition Making referrals to specialized services when necessary Your efforts will contribute to decreasing trauma impacts, improving family support, and fostering inter-service collaboration.
