The position exists to provide a specialist case management service to young people who are considered to be at risk and residing in family based settings, independently or in home-based care placements. Managing a caseload of generally disengaged, complex, high-risk youth involved with Child Protection.
The service aims to strengthen the resilience of young people who are subject to protective involvement by providing a specialised, outreach based case management service. The young people who access the service are often experiencing multiple complicating factors such as:
drug and alcohol use and/or misuse.mental and/or physical health concerns.Are homeless or at risk of homelessness,Have experienced the impacts of violence and/or use violencerelationship breakdown and/or difficulties with peers and family.experience significant issues in relation to their personal safety and wellbeing.The skills you will use day to day: Your knowledge of Best Interest Principles as outlined within the Children, Youth and Families Act 2005.Conduct assessments and safety planning.Case contracting and intensive case management.Direct client engagement to progress Case Plan direction.Generate positive outcomes in relation to safety, stability and development.Use a range of active engagement strategies with vulnerable and at-risk young people and their families.Crisis management with conducting extensive assessments to identify risk and coordinate safety plans accordingly.Supporting the coordination and communication with relevant stakeholders to meet the needs of the client.Daily documentation around case management tasks and formal report writing.What makes this such a great role for you: You get a salary starting at around $94,000 plus super.Salary packaging is available up to $15,900 plus an entertainment card.SCHCADS award level 5.Negotiable through the increments dependent upon experience.Work from our great office space in Bendigo.We take the Health and Wellbeing of our people seriously offering a Well Worth It!
