Role Description: The Integrated Team Care (ITC) Coordinator will be responsible for supporting the Integrated Team Care (ITC) Workforce (Care Coordinators and Outreach Workers) employed in AMSANT member services who receive ITC funding. The ITC program aims to ensure a focus on Aboriginal health and chronic disease, contribute to closing the gap in life expectancy and aims to improve care integration and coordination.
The ITC Project Officer will work in close collaboration with the PHN, AMSANT, organisations that employ ITC Care Coordinators and Outreach workers and the Care Coordinators and Outreach Workers themselves to actively support opportunities that enhance the improvement of Indigenous health through care coordination.
Responsibilities: Provide support, training opportunities and resources to AMSANT members and their chronic conditions workforce.Coordinate meetings and forums with AMSANT members and relevant stakeholders on chronic disease management.Provide input into key chronic disease and workforce initiatives that are relevant and appropriate to AMSANT and its members.Plan and coordinate the annual Integrated Team Care (ITC) Forum in partnership with the NTPHN and other AMSANT staff.Represent AMSANT at key external meetings and forums as requested by team manager.Provide succinct and timely reports as required for the ITC Program.Undertake other duties appropriate to the level of the position as directed to meet work unit and AMSANT objectives.Participate in work unit and programming meetings.Selection Requirements (Qualifications & Attributes) ESSENTIAL Sound knowledge and understanding of chronic conditions in Aboriginal health, including a sound understanding of the social determinants of health and how these affect chronic conditions management.Demonstrate knowledge and understanding of Aboriginal and Torres Strait Islander peoples, cultures, histories, politics and principles of Aboriginal community controlled health sector.Tertiary qualification and/or experience in Health or Human Services or the equivalent level of expertise in the Aboriginal Community Controlled Organisation sector. This includes across the health sector, or member-based organisations gained from a combination of experience, training, skills and personal attributes.Proven ability to coordinate projects and simultaneous activities, including the ability to work independently to coordinate and deliver multiple projects with multi-disciplinary teams.Ability to effectively engage with a diverse range of organisations and individuals at all levels, informal and linking trainees to professional support.Ability to regularly travel within the NT to regional and remote communities by road or light aircraft.DESIRABLE Previous experience working in Aboriginal primary health care in the NT.Knowledge and experience in health policy, specifically in chronic conditions.Knowledge of and understanding of the work of AMSANT.Additional information: Original qualifications must be available for viewing and copies to be kept on personnel file.Applications must have an Ochre Card and a Current Police Check and/or ability to obtain these prior to commencement.The COVID-19 Safety Management Plan and other COVID – 19 related information Policy and Procedure require all AMSANT employees to have had a third COVID-19 Vaccination.
