About Us:

We aim to improve health outcomes by offering a comprehensive range of services, including allied health, mental health, chronic disease management, public health, health promotion, and more. We are now looking for an Integrated Health Manager to lead a dedicated team in delivering coordinated care and integrated health services across multiple community health programs.
Role:
The Integrated Health Manager is responsible for leading a multi-disciplinary team that includes Allied Health Professionals, Clinicians, Community Workers, and Public Health Workers. In this role, you will ensure the effective coordination of care, public health initiatives, and chronic disease management programs, all within an integrated approach in a Primary Health Care setting. You will be instrumental in supporting early intervention, treatment, and health management programs to achieve improved outcomes across the community.
Key Responsibilities: Leadership & Team Management: Oversee and manage the Integrated Health team, ensuring all staff follow SHSAC policies, procedures, and funding agreements.Program Management: Manage the Integrated Health Team's portfolio, ensuring KPIs, outputs, and outcomes are met as per funder guidelines.Planning & Evaluation: Lead planning, implementation, monitoring, and evaluation of team programs and activities.Community Engagement: Work with local communities to implement health promotion and early detection initiatives, focusing on prevention and well-being.Allied Health Services: Monitor and support the delivery of allied health services, including SONT, MOICD, RPHS, Outreach, and other programs.Collaborative Care: Foster a collaborative approach within the team, ensuring a unified and effective delivery of primary health care services.Reporting & Documentation: Ensure timely and accurate submission of all required reports, program goals, and outcomes.Team Development: Facilitate staff development, training, and opportunities for career progression within the organisation.Change Management: Address and lead change management initiatives within the team to adapt to evolving needs and services.Other Duties: Perform other duties as assigned by the manager.What You'll Need to Succeed: Tertiary qualification and/or experience in the Primary Health Care sector.Current First Aid Certificate.Non-adverse National Criminal History Check.Current OCHRE clearance.Acceptable vaccination status to work with vulnerable people in the Northern Territory.Right to work in Australia.What You'll Bring to the Team: Excellent organisational, project, and financial management skills.Experience designing, planning, implementing, and evaluating community-driven programs in remote Aboriginal or low socio-economic communities.Proven track record in managing multidisciplinary teams.Experience in change management within health or community services.Familiarity with the Northern Territory health system and services.Strong understanding of Aboriginal community-controlled health services and cultural safety.Knowledge of Primary Health Care principles, including social determinants of health, particularly in a remote Aboriginal community setting.Familiarity with the Medicare system or willingness to obtain knowledge.Why Join Us?
