Join our Integrated Health Care team to support the PHN in further developing key partnerships to deliver integrated and innovative solutions to increase the equity of access to primary health care.
About the Role
The Integrated Health Care Officers provide primary care leadership and support to projects supporting integration activities across the PHN region.
The role will foster a 'whole of system' approach and encompass the influencing determinants of health that are cross-sectoral and across geographical boundaries.
The Integrated Health Care Officers will work closely with stakeholders and partners, including but not limited to primary care providers, Hunter New England Local Health District (HNELHD), Central Coast Local Health District (CCLHD), NSW Ambulance, local Aboriginal Medical Services, aged care providers, and other external organisations.
The Integrated Health Care Officer is responsible for building strong collaborative relationships with key internal and external stakeholders and partners to support the development, implementation and evaluation of new models of care.
Newcastle candidates please note regular travel to the New England region is a requirement of this position.
The successful candidate will have:
Relevant tertiary qualifications in a health-related field and/or extensive relevant experience in a primary health care environment or extensive experience working in project teams.Ability to build and maintain strong stakeholder relationships liaising with a range of internal and external stakeholders and organisations to influence change preferably in a health setting.Demonstrated ability to work both independently and in a collaborative team environment on projects and activities with proven ability to deliver high quality outcomes in agreed timeframes.Excellent interpersonal skills, with well-developed written communication skills including writing project reports and management briefings and development of project materials to different audiences.How to apply: Address the selection criteria (both essential and desirable) in your cover letter.

*Salary: This role is classified as I – M within our Enterprise Agreement and the rate will be dependent on skills and experience/qualifications.
New appointments are generally offered between grade I – K.
