This new and dynamic role will be working with the care managers of the home care package, CHSP, STRC team and all relevant parties. To be the first stage of our Home care package, CHSP, TCP, DVA and STRC clients when they wish to sign up to Calvary Community care. Working with a holistic approach to help them to develop their care plan and services in their home within their home care package, CHSP, STRC, DVA and TCP funding.Your main duties will include:Initial contact and relationship developing with new Home care package, CHSP, STRC, TCP and DVA clientsCompletion of New client comprehensive assessment and paperwork working with clients especially CHSP, STRC, DVA and TCP.
