We are looking for an enthusiastic and compassionate Intake Officer to join our Home and Community Care team. Role Summary:
The Intake Officer provides a centralised point of entry for all program referrals, responsible for managing and processing referrals across all geographical areas of the organisation. This role is crucial in delivering a personalised service and building effective relationships with internal and external clients, as well as liaising with staff, consumers, and their families.
Key Responsibilities: Manage and coordinate all program referrals, ensuring they are processed via the appropriate channels (e.g. MAC, VHC Portal)Liaise with Allied Health Manager and Community Care Managers to ensure accurate and timely service deliveryProvide information regarding service availability and costs to clients and referrersMaintain accurate records and ensure compliance with relevant policies and guidelinesEnsure all service delivery information is up to date and accurate in relevant systems (e.g., MAC portal, Carelink)Proactively engage in promoting community care services and enhancing support for the local community.Support a culture of teamwork, quality service, and respect for diversityKnowledge & Experience: Practical experience working in a busy office environment, ideally with multidisciplinary teamsExperience or understanding of working with older people, people with disabilities, or knowledge of Consumer Directed Care and Home Care GuidelinesExperience with electronic rostering systems is highly regardedCertificate III in Business Administration or equivalent qualification or experienceWhat We Offer: Competitive salary plus superannuationSalary packaging (up to $15,900) to increase your take-home paySupportive and collaborative team environmentOpportunities for professional development and continuous improvementTo be eligible to apply for this position you must be eligible to work in Australia, be willing to undertake a criminal record check in accordance with the requirements of the Aged Care Act 1997.
