Intake And Administration Officer - Community Health

A new opportunity has arisen for experienced Administration Officers to join our Central Intake team and provide professional administration support to our Community Health services.
The Contract: Ongoing Part-Time, 48.00 hours per fortnight The Position: The Central Intake and Care Coordination Unit provides coordination to the BRHS Health Independence Program.
This program encompasses many of the services that deliver care to support clients both in the community and at home and that also support clients with transition from hospital to home.
The key role of the Intake and Administration Officer is to identify our client's specific needs by performing an initial needs assessment to ensure access and support from the right service at the right time is delivered to the client, to enable the best outcome to be achieved.

* Proven ability to provide a consistent and high level of person-centred customer service.
* Ability to manage data efficiently and accurately with an aptitude for interpreting and reporting information appropriately.
