You will provide rapid crisis intervention and interim support to people who are homeless or at risk of homelessness to access affordable accommodation to prevent their return into the homelessness service system.You will successfullyInitial assessment is developed through understanding of client's needs, risks, and vulnerabilities Undertake quality assessments and provide short-term/interim support Complete accurate reports, case notes and files to a high professional standardProvide linkages into community supports and programs for clients who are homeless or at risk of homelessness with multiple and complex needsDevelop effective relationships with clients that encourage self-determination and participationThis is a permanent part-time position for 19 hours per week, reporting to the Horsham/Hamilton Team Leader, located in Hamilton, VIC.
