Salary: $49.28 p/hr + super + $2.78 p/hr regional allowance

About the Opportunity: The Indigenous Health Project Officer (IHPO) has a policy and leadership role and, as a team leader, ensures there is a focus on Indigenous health, and improved integration of care across the Goldfields region. This work includes needs assessment and planning, developing multi-program approaches and cross-sector linkages, and supporting both Outreach Workers and Care Coordinators.
The IHPO leads ITC staff to work across the Indigenous and mainstream primary care sectors, to assist Aboriginal and Torres Strait Islander people to obtain primary health care as required; oversee care coordination services to eligible Aboriginal and Torres Strait Islander people with chronic disease who require coordinated, multidisciplinary care; and ensure effective treatment pathways to improve access for Aboriginal and Torres Strait Islander people to culturally appropriate mainstream primary care.
Requirements of the Role: Increase awareness of and maximise links between services for Aboriginal and Torres Strait Islander people, including those provided by Commonwealth and state governments, Aboriginal Medical Services, and other organisations; Facilitate working relationships and communication exchange between mainstream organisations, Aboriginal Medical Services, and their peak bodies; Work with local stakeholders to develop and implement strategies to improve access to mainstream primary care for Aboriginal and Torres Strait Islander people. Develop, implement, and evaluate strategies to improve the capacity of mainstream primary care providers to deliver culturally appropriate primary care services to Aboriginal and Torres Strait Islander people, at a local, community, and regional level, including: self-identification; uptake of Aboriginal and Torres Strait Islander specific MBS items including item 715 - Health Assessments for Aboriginal and Torres Strait Islander People, care planning and follow up items; increase awareness and understanding of the COAG targets to close the gap in Indigenous disadvantage; and collaborate with local Aboriginal and mainstream health services in a partnership approach for the delivery of primary care services. About you: Commitment to upholding HOPE's vision, mission and values. Experience in working with Aboriginal people and in culturally secure ways. High level of written communication skills, including report writing. High level of interpersonal and verbal communication skills, including negotiation skills and the ability to liaise with people in a variety of contexts. Qualifications, skills and experience are not specified for the IHPO role. It is expected that IHPOs will have the qualifications and/or skills and experience in working with Aboriginal and Torres Strait Islander people required for the performance of the roles and responsibilities. Demonstrated experience leading a team to provide a coordinated approach to service delivery. Demonstrated ability to work with government, non-government, industry and community stakeholders to coordinate service delivery. Aboriginal and Torres Strait Islander people should be engaged to work as IHPOs where possible. IHPOs are expected to work as team leaders and support Care Coordinators and Outreach Workers. An understanding of and adherence to working within personal and professional boundaries. Willingness to drive across the Goldfields Region. Desirable criteria: Tertiary degree qualifications in health, education, social science, or approved equivalent, with demonstrated experience.
