In Home Care Case Coordinator - Aged CareComfort Keepers - South are looking for an experienced HCP Case Coordinator to join our Coordination team.
About Us Comfort Keepers - South is a leading Government approved Aged Care provider delivering a wide range of services to WA older clients for over 20 years.
Our office is based in Applecross, and we service clients in their own home, from south of the river covering the surrounding areas of Rockingham, Fremantle, Cannington and Vic Park.

About You You have proven case-management skills, are highly motivated and passionate about delivering high quality person-centre care.
You will be case managing and playing a key role in all aspects of our client's care and will therefore need to be flexible and innovative with highly developed problem-solving skills and be responsible for the management and co-ordination of a portfolio of private and government funded programs in accordance with Quality Standards, relevant legislation and Quality Indicators.

To be successful in this role you will need to demonstrate an understanding of person-centre care, focusing on enabling independence for your clients.
Key Responsibilities: Building positive long-term relationships with clients, their families, allied health professionals and community support workers.Working closely with clients and their families to develop detailed and person-centred care plans that deliver evidence-based outcomes.Screen and identify client care / clinical needs, solve issues with realistic, practicable solutions.Work with the Comfort Keepers broader team to ensure a high level of service is delivered to clients.Managing, monitoring, and reporting on client packages in line with the budgets that have been set.Proactively promote Comfort Keepers to potential clients and a wide range of referral partners.Demonstrated understanding of the guidelines of government funded Home Care Packages and the inclusions and exclusions framework.Organise daily schedule to meet the needs of client assessments, reviews and updates of documentation to meet timelines and client requirements effectively and efficiently.Comply with relevant legislation, policies and procedures relating to the delivery of Home Care services.Develop and maintain a wide knowledge of resources available to support clients to optimise their package funds.Manage client concerns, complaints or other feedback in a timely and professional manner as per Comfort Keepers South Feedback and Complaints policy.Skills and Experience: Be driven to achieve the best customer service and experience for our clients and their families.Highly developed communication, organisation and problem-solving skills.Solution focused and proactive with providing effective solutions for our clients.Excellent assessment and documentation skills with high-level attention to detail.Previous experience in Aged Care Home Care Package Coordination or similar.Proficient in Microsoft Office software.Selection Criteria The ideal candidate will ideally have these qualities:
A positive outlook and a proactive attitude.My Aged Care sector experience and be familiar with working within the Home Care Package framework and terms of business.Have at least 2 years' experience in a case management role working with older clients in the community providing support to stay at home.Demonstrated ability to operate within organisational and legislative guidelines, procedures and policies.Strong computer skills, with experience in Microsoft applications and Client Record Management systems.Ability to work independently as well as a part of a team.Demonstrated ability to independently plan, manage and organise own workload in a complex environment.Reliable vehicle and current driver's licence. #J-18808-Ljbffr
