Position Type: Continuing Contingent* part-time appointment (0.6 FTE, 21 hours per week)
Salary: Level 8/9: $113,063 - $140,279 (pro-rata) + 17% Superannuation
Location: The position is located in the Melbourne CBD. Remote working will be considered in line with UOW policies and business needs.
An exciting opportunity exists for a talented palliative and/or aged care health professional with experience and effective skills in communication, education, health information, research, and quality improvement.
AHOC is a research consortium within the Faculty of Science, Medicine and Health (SMAH) which aims to improve services and outcomes for patients through benchmarking of care and treatment and supporting service improvement. AHOC generates robust evidence through its provision of clinical quality registries in rehabilitation, pain, and palliative care. The registries provide evidence to guide treatment and care for individual patients, measure outcomes, and deliver benchmarking for the relevant health sector.
The Palliative Aged Care Outcomes Program (PACOP) is one of four outcomes programs which make up the Australasian Health Outcomes Consortium in the Faculty of Science, Medicine and Health (SMAH) and is funded by the Australian Government Department of Health and Aged Care. PACOP is dedicated to improving the care and outcomes of all Australians living in aged care homes with a particular focus on those who require palliative care and are approaching the end of their life. PACOP works collaboratively with aged care homes and organisations throughout Australia to implement the program to support continuous improvements in residents' end of life outcomes.
As an improvement facilitator, you will play a key role in the PACOP team. This position involves leading and managing PACOP activities, contributing to clinical and professional practice changes within participating aged care homes based on the outcomes of PACOP's research activity.
Key Responsibilities: Lead and manage PACOP activities. Contribute to clinical and professional practice changes within participating aged care homes. Collect, analyse, and interpret data to enhance clinical practice. Implement change management and quality improvement principles within healthcare settings. To be successful, you will be suitably qualified with extensive relevant experience working in palliative care, aged care, and/or quality improvement in health care or a combination of education/training/experience deemed equivalent. You must demonstrate strong project management skills, including the ability to plan ahead, prioritise tasks, and meet deadlines in a busy environment. Additionally, you should have a solid understanding of change management and quality improvement principles within healthcare settings, as well as an understanding of the use of data, databases, and IT systems to capture clinical information.
