Immediate Housing Response Case Manager (Gladstone)Roseberry Qld Immediate Housing Response (IHR) Case Manager supports people in the community who are experiencing, or at imminent risk of homelessness. The program supports singles and couples aged 16 years and above, who are identified as needing assistance to establish and sustain stable accommodation, alongside pro-active assistance to transition to longer-term housing options, therefore breaking the homelessness cycle.The position aims to ensure that people have access to emergency brokerage for essential tenancy or temporary accommodation commencement and/or sustainment items alongside case management.Key Responsibilities/Outcomes:Respond to initial enquiries and referrals within 72 hours of receiving the information.Case manage the required number of clients as per the service agreement between RQ and the Department of Housing and Public Works and as negotiated with the Team Manager in consultation with the General Manager.Provide immediate short-term temporary accommodation for people experiencing homelessness or at imminent risk of homelessness in Queensland, with proactive support services to facilitate transition to safe and secure housing through home ownership, the private rental market, or the social housing system.Actively work to enhance the dignity, self-esteem, and independence of clients and to ensure that service delivery is inclusive of all people, including Aboriginal and Torres Strait Islander peoples, culturally and linguistically diverse people.Build rapport and engage with clients in a professional and competent manner, always encouraging and assisting young people to take ownership of their decision-making, their actions, and subsequent consequences.Maintain client records, case records, and personal information in a confidential, safe, and secure manner as per the RQ Policies and Procedures, funding requirements, and privacy legislation.At all times be alert to changes in behaviour and physical condition of clients and where changes are noticed, advise the Team Manager immediately; at all times ensure that duty of care responsibilities are enacted.Provide assistance to clients using a case management approach ensuring a high-quality service following RQ principles, forms, and processes, for example, use of case notes, support plans, and standardised file structure.Provide one-on-one assistance and practical support to clients in order for them to establish and maintain independent, secure, and stable housing by utilising a case management approach including: providing immediate short-term temporary accommodation, assisting with stabilisation of tenancy and transition to longer-term accommodation, coordinating case management, and distribution of brokerage.Provision of information and advice regarding stable housing options including a working knowledge of the Housing and Homelessness Services and tenancy legislation.Making referrals to appropriate and relevant agencies.Current Blue Card – issued through Blue Card Services.Current Driver's License.Youth Worker qualification or part completion of behavioural/social science certificate/diploma and/or significant life experience would be viewed favourably.For more information, call our office on 07 49720*** and speak to our Youth & Family Services Manager.
