Working across Northern Sydney, you will facilitate non-clinical mental health / psychoeducational groups, case management support to consumers accessing our Psychosocial Support service. Work on individual recovery goals, which may include accessing mental health/AOD support, exploring housing options, enhancing financial stability, and referral to other appropriate service where required.
Your key responsibilities will be to: Develop and deliver psychoeducational group programs with consumers and staff and maintain up-to-date records. Develop and maintain relationships with internal and external stakeholders. Provide accurate record keeping and updating of case notes and reports. Uphold the program values and standards to ensure a positive outcome. Identify potential improvements in service provision to participants and participate in implementing all quality program activities. Provide face to face case management support for clients by creating holistic and strength-based individualised support plans using SMART goals including referrals to supplementary services. Requirements for Success A relevant qualification in Community Services, Social Work or related field. Well-developed written and verbal communication. Driver's Licence. First Aid Certificate (or willing to obtain). Before starting work with us, you will need to undertake a national police history check, qualifications, referee checks and WWCC.
