Housing Engagement Coordinator

Opportunity The Housing Engagement Coordinator engages with customers to match home and living support options for new and existing Ability WA customers. This role involves working with external stakeholders to help customers navigate their living choices, offering high-quality support from enquiry stage to moving in. We are looking for a passionate individual who can build strong referral networks, establish connections, and deliver a positive experience for our customers.
This is a Permanent Full Time role based in our Coolbinia Hub. About the Position As a Housing Engagement Coordinator, you will:
Work with customers to achieve their home and living goals within Ability WA's home and living optionsMaintain strong relationships with internal stakeholders including but not limited to Community Services, Corporate Services, Therapy Services and the Customer Engagement and Contact TeamsPromote Ability WA as a preferred home and living supports service provider at events e.g. ExposEngage with SDA partners and key stakeholders in the establishment of new SDA homes.Liaise with internal and external therapy, and Support Coordination providersWork with Community Services teams to ensure the timely transition into Ability WA's homes and safe service handoverPreparation of intake assessment documentation, customer snapshots, Supported Independent Living Quotes, development of Rosters of Care (ROC) and house rosters, and customer risk assessmentsAbout You Sound understanding of the disability sector & NDISStrong communication and interpersonal skillsAbility to develop and maintain relationships with key stakeholdersDemonstrated experience in home and living support services in the disability sector or relevant experienceStrong attention to detail and committed to continuous improvementWell-developed computer skills and strong proficiency with the MS Office SuiteRelevant tertiary qualification is desirableCurrent (or willing to attain): Working with Children Check, National Police Clearance of no older than 6 months, Current Driver's Licence, NDIS Workers Orientation Module, NDIS Worker Screening check, COVID-19 Vaccination Certificate and Current First Aid CertificateAt Ability WA, we care about finding the right people to join us hence encourage you to apply even if you don't meet 100% of our selection requirements.
